New Patient Appointments

We accept new patients and referrals. Please come a few minutes early to fill out a patient history form. On your initial visit, please bring the following items:

• Identification
• Insurance card(s)
• Written referral form, if required
• List of medications you are taking
• Pertinent test results

Also, be prepared to share your medical history with our physicians. This will include any medical conditions you have or may have had in the past, surgical history, any medications (prescription or over-the-counter) you are currently taking (include name, dosage and frequency), and any allergies you have to medications or foods.

Insurance

We accept most major medical insurances. Some of which are Medicare (West Virginia and Virginia), Blue Shield, Aetna, PEIA, UMWA, Carelink, Southern Health, MAMSI and Anthem.
