Thomas S. Kowalkowski, MD, FACS Board Certified Urologist
Dr. Thomas S. Kowalkowski was born in Norristown, Pennsylvania. He received his undergraduate degree in chemistry from Franklin and Marshall College in Lancaster, Pennsylvania and his medical degree at Hahnemann University in Philadelphia. Dr. Kowalkowski completed his internships and residencies in general surgery and urology at West Virginia University.

He was board certified by the American Board of Urology in 1993 and recertifed in 2001. He is also a Fellow of the American College of Surgeons.

David Farbod Meriwether, MD, FACS Board Certified Urologist
Dr. David F. Meriwether was born in Washington, District of Columbia. He received his undergraduate degree in biology from Trinity College, Hartford, Connecticut and his medical degree from the University of Cincinnati, Ohio. He completed two years of general surgery residency at Vanderbilt University in Nashville, Tennessee. He then completed four years of urology and renal transplant at the University of Pittsburgh in Pittsburgh, Pennsylvania.
Dr. Meriwether is a member of the following medical associations and societies:
He is past president of the Greenbrier Valley Medical Society and past chief of staff of Greenbrier Valley Medical Center.
He was board certified by the American Board of Urology in 1988 and recertified in 1996. He was inducted as a Fellow of the American College of Surgeons in 1989.

Kyle Fredrick Fort, MD, FACS Board Certified Urologist
Dr. Kyle Fredrick Fort was born in Indianapolis, Indiana. He received his undergraduate degree in chemistry and pre-med from Depauw University in Greencastle, Indiana. He was a member of phi beta kappa and a starter and co-captain of the varsity basketball team. He completed his medical degree at Indiana University School of Medicine in Indianapolis and his internship at Medical College of Ohio where he also completed his residency in adult and pediatric urology.
Dr. Fort is a member of the following medical associations and societies:
Dr. Fort is past president of the West Virginia Chapter of the American College of Surgeons and past president of the Greenbrier Valley Medical Society. He is past Chief of the Department of Surgery as well as past Chief of Staff of the Greenbrier Valley Medical Center.
He was board certified by the American Board of Urology in 1985 and recertified in 1993 and recertified again in 2004. He was inducted as a Fellow of the American College of Surgeons in 1987.
His article titled "Stent versus Non-Stent for Uncomplicated Ureteroneocystostomies," was published in the Journal of Urology in March 1983.

Joseph Mouchizadeh, MD, FACS (aka Moshy) Board Certified Urologist
Dr. Joseph Moshy was born in London, England. He received his medical degree from St. Mary's Hospital, University of London. He completed four years in England and came to the United States and completed his urology residency at the University of New Mexico in Albuquerque, New Mexico. He spent ten years in practice in California and joined this practice in May 2002.

Dr. Moshy was board certified by the American Board of Urology in 1994 and recertifed in 2004. He was inducted as a Fellow of the American College of Surgeons in 2004.

James E. Cauley, M.D., FACS Board Certified Urologist
Dr. James E. Cauley, M.D. was born in San Diego, California to a career naval man. With his father's retirement the family settled in Fort Wayne, Indiana. Dr. Cauley went to Cumberland College for an undergraduate degree in Chemistry and Mathematics. His medical education was at Indiana University School of Medicine in Indianapolis, Indiana. Dr. Cauley then completed a General Surgery internship at the Medical College of Wisconsin in Milwaukee, Wisconsin. He would continue at the Medical College of Wisconsin to complete a urology residency.
Dr. Cauley is a member of the following organizations or societies:
Dr. Cauley is past secretary and president of the Winnebago County Medical Society and the Dodge County Medical Society. He is past president of the Department of Surgery, Mercy Medical Center, Oshkosh, Wisconsin. Dr. Cauley was board certified by the American Board of Urology in 1988 and recertified in 2006. Dr. Cauley became a Diplomat of the Fellow American College of Surgeons in 1989.

Colleen J. Meriwether, D.O. Board Certified Family Practitioner
Doctor Colleen J. Meriwether was born in Albany, Georgia. She received her undergraduate degree as a physician assistant at the Medical College of Georgia and her medical degree at the West Virginia School of Osteopathic Medicine, Lewisburg, WV. She completed a one year Internship and two years of Residency at the Greenbrier Valley Medical Center, Ronceverte, WV. Dr. Meriwether is a member of the American Osteopathic Association and past president of the Greenbrier Valley Medical Society. She was board certified in January 1998 and recertified in October 2005.

 | Tyler Vaughn, MPAS, PA-C Certified Physician Assistant
Tyler Vaughn is the physician assistant for Dr. Kyle Fort. Tyler was born in Parkersburg, West Virginia, and reared in Eagle Rock, Virginia. He now resides in Lewisburg, West Virginia. He received his bachelor's degree in business and economics from Emory and Henry College in 1992, and his master's degree in athletic training (sports medicine) from West Virginia University in 1995. He earned another bachelor's degree in physician assistant studies from the Interservice Physician Assistant Program (U.S. Army) in 2003, and his master's of physician assistant studies (MPAS) from the University of Nebraska Medical Center in 2004.
Mr. Vaughn has been an active member of the 2nd Battalion, 19th Special Forces Group (Green Berets) since 1995, where he served as a Special Forces Medical Sergeant until commissioning in 2003. He now serves as one of the Battalion physician assistants.
Coincidentally, Mr. Vaughn is also Dr. Fort's son-in-law. He joined our practice in June 2003.
